Our client is a local organization that provides care and support for the elderly and regularly engages in community services. They aspire to ensure quality and high standard services focusing on the importance of quality eldercare.

With their ever-growing expansion, they are looking to hire Eldercare and Community Outreach Associate to assist with their daily operations.

• Review referrals for befriending and buddying services and conduct home visits for clients referred for the service.  

• Update IRMS on status of referral and ensure that there is proper handover of clients who reject or does not meet the criteria of the service.  

• Deliver high level of customer service and maintain frequent engagement with seniors and community partners. 
• Support targeted outreach and provide quality befriending or buddying services to socially isolated seniors as guided by the service requirements. 

• Provide information and referral services to seniors or the community in need at the Active Ageing Centre. 

• Support in Active Ageing activities and outings organised for the seniors.   
• Maintain proper records, documentation on the check calls, and information and referral of the engaged seniors.  

• Support and assist the Center Supervisor in compiling data to meet reporting requirements. 

• Work closely with volunteers for befriending and buddying. Ensure that volunteers meet service requirements and there are no service lapses.  

• Any other responsibilities and duties the Center Supervisor/Senior Management assign.

If you’re someone with the following, let’s chat!

• Bachelor’s degree or diploma from an accredited University or Polytechnic. 
• Min. 1 year working experience in the field.

Additional details

• Work Locations: Braddel Heights, Serangoon, Shunfu, Yew Tee, Kreta Ayer, Eunos, Changi and Simei.
• 5.5 Workday, Monday to Saturday.
• Work timing between 8.30am to 5.30pm.
• Uniform provided.
